Left ventricular reentry arrhythmia A case report.

Abstract
Idiopathic ventricular tachycardia (VT) encompasses a heterogeneous group of ventricular tachycardias, in which structural heart disease cannot be demonstrated (dysplasia of the right ventricle, mitral valve prolapse, certain cardiomyopathies), metabolic-electrolyte alterations, presence of QT interval long or Brugada syndrome. In general, VT is considered idiopathic in 11%. Idiopathic left fascicular VTs are a rare form of VT, representing 10 to 15%, being more common in young adults (15 to 40 years), of which between 60% and 80% are men. The majority form of these tachycardias involves the posterior fascicle and is therefore called posterior fascicular VT, with morphology of right bundle branch block and superior axis. Although pharmacological therapy can be effective in a good percentage of patients, ablation has a high success rate, low complication rate, and long-term recurrence. Overall success rates for idiopathic fascicular VT ablation range between 70% and 90% relative to long-term follow-up. A clinical case is presented at the annual congress of the Alcívar Hospital.
